The deadline for claiming a Risk Management Credit for your 2022 premium is September 15, 2021.
The online declaration form appears below, and must be filled out by this deadline for you to receive the credit. You are
eligible to claim a 2022 premium credit of $50 per program (to a maximum of $100) for attendance at approved
programs offered between September 16, 2020 and September 15, 2021.
You can claim the credit for reviewing an archived replay of an approved CPD program,
provided you watch or listen to the entire program and have a copy of the program materials. On the CPD declaration form
you should claim credit for a self-study review.
You are also eligible for the Premium Credit if you chair or speak at a qualifying program, provided you attend the
entire program. The Premium Credit is applicable to Standard coverage, and cannot be applied to Run-off or Excess policy
Approved programs offered between September 16 2020 and September 15 2021 will be
eligible for the 2022 Premium Credit. Risk Management Credits may not be carried forward. Unclaimed
credits for attendance at qualifying programs offered before September 15, 2020 are now lost.

You must fill in all the information requested on this form.
The Risk Management Credits will automatically appear on the invoice you will receive with your 2022 insurance policy.
